I've never observed any paper grain with my HP ScanJet 3c at either
300 or 600 DPI as strict B&W (one bit).  But it's my understanding
that the ScanJet 3c (and 4c, 6100) do some image processing in the
scanner.

Image (as opposed to document) scanning always picks up the grain
of the underlying medium, or it would miss highlight details.

After all, it's a subjective judgement whether the paper grain
in the highlights is part of the image or not.  ;-)
