My concern would be that some sort of data corruption is going on, and
it's present in the rest of the disk but for some reason isn't noticeable.
If possible, copy a disk image to a real floppy, then copy the new floppy
back to Windows. If you get I/O errors, you've identified one problem.
If not, you can try booting the new image with an emulator. If that fails
the same way, we can compare the images to see what changed.
