I was under the impression that you needed a formatted
disk for ADT to be written to. Hence the sending of the
DOS.DMP file first. At the end of the ADTxx.DMP file is
a BSAVE command that writes the file to disk. What you
should do is follow the directions above only send the
DOS.DMP file to your Apple II and before anything else
insert a blank disk and type INIT HELLO or just INIT
works too I think and you'll have a disk with DOS 3.3
on it. Now you can dump ADT to your Apple II and it will
write the program to that disk. Now INIT another disk
and use ADT to transfer a DSK image and write it to the
new disk and so on.  :o)
