It works in both actually.

This card replaces the 9334 located on the far right side of the II/II+ board Row F (same row as ROMs).

It is set up so you can't plug them in to the wrong place. There is only one correct setup that works.

Yes, if you put the small board where your 9334 is on the main board your system will work fine. In 40 column mode you use the
standard video out port on the Apple IIplus, in 80 column mode you use the Videx video out port. I remember my dad building me a
video switch for this setup long before there was anything on the market for it but you can get a video selector switch from Radio
Shack that will do the trick for you.
