Make sure you have a 65c02/65816 equipped Apple (enhanced IIe,
IIc, IIc+ or IIgs) to run ProDOS 8 v2.x.

Yes. Note that a Smartport card in slot 5 will still try to use
slot 2 for drives 3 & 4, then it will use the table to assign the
remaining drives.
Your table of drives looks correct to me.

Correct. ProDOS is only concerned with disk controller cards
during this phase of initialisation so ignores
serial/printer/mouse/80col/clock etc cards. BASIC.SYSTEM
on the other hand ignores the virtual drives so if you do PR#2
it will call the serial card in slot 2 not try to boot from
a remapped drive.
