Thanks to everyone who replied. Since trying to get this card running I have had
the very good fortune to buy another Apple II and it had a genuine Microsoft
softcard and a Videx 80 column card inside, and some genuine Microsoft CP/M
system disks.

With all this extra stuff I was able to discover that my clone Z80 card is
probably not a very close copy of the genuine card. Its made by "U
Microcomputers Ltd" and is labelled "UZ80 Processor for Apple II" and although
the layout of the chips is very close to the microsoft card it doesn't have the
option switches on it.

My new Z80 card boots up in 80 column mode AND 40 column mode if I unplug the
Videx card.
