Basically, the card was connected to a keyboard substitute for use
by physically handicapped people. The cards aren't that uncommon but
the "keyboards" are.
The Unicorn keyboards are programable boards with 1" touch ensitive
squares. If need be groups of quares can be programed to output each
keystroke.

An I/O box is also needed but that appears to be missing

This is one of the IIgs versions, there were IIe versions as well.
