How to create .dsk images

I have a bunch of 5.25" Apple II floppies that I want to copy to .dsk
images. The disks are a mix of Pascal, CP/M, DOS 3.3, and ProDos types, and
I want to move the images to my PC so that I can then move them to some
place like the Asimov FTP site. I have ADT set up and working on my PC and
Apple but I've only used it to create floppies from .dsk images -I've never
created a .dsk image.
What is the best way to create these .dsk images and move them to my PC?
Some of them might be copy protected but I'm not sure if that's a problem.
