First try something easy, hold down the shift key while booting the Mac. This sometimes lets you bypass the password protection.

If not then try

If you have a set of 7.5 floppies then there's likely two disks, Disk Tools 1 & Disk Tools 2. You should be able to boot off one of those, I can't recall which one works for this particular Mac. To do that put the floppy in the drive before starting it up. After starting off the floppy, you should be able to either trash the System folder on the HD, erase the HD from the Special menu or initialize the HD using the HD SC Setup program on the floppy.
