Beneath Apple DOS, by Worth & Lechner.  Just ignore their claim that
there is a clock bit between every pair of data bits, that's wrong.
The clock bits are how FM encoding works, not GCR.  The whole point
of GCR is that you don't NEED clock bits.

But all the rest of their discription of the Apple disk format is
good.
