The manual(1980) says "You may plug the card into any unused slot(except 
slot zero)"

The 4 switches 
S1-1 Address offset  (Normally off)
[ lets Z80 address contiguous from $0000-$DFFF when actually using Apple 
memory $1000-FFFF (avoiding zero page $0000-$0FFF and $C000-$CFFF pages)]
when on memeory addresses are not shifted.

S1-2 Z80 DMA Enable (usually off)
when off it preserves the interrupt daisy chain

S1-3 Non-Mask Int (usually off)
S1-4 Z80 Interrupts (usually off)
when on the Z80 will be able to respond to interrups in the Apple.
