In a II+ the Monitor ROM is the F8 ROM. The other 5 contain Applesoft Basic.
In the II the F8 is also the Monitor but it's not an autostart. You get
garbage on your screen when you power up and have to do a reset then a
Control-B Return to get into basic. In the Apple II Integer basic
resides
in the F0, E8 and E0 ROMs. D8 & D0 are empty unless you have the
programmers
ROM.

All II and II+ had a maximum of 48k on the motherboard. Minimum RAM any
II+
came with is 16k. With the II it depends, earlier ones came with as
little
as 4k, later ones had a minimum of 16k.
Slot 0 is almost always used for a 16k card often referred to as a
Language
card. The 16k card brought the II or II+ up to 64k. It was called a
language
card because that's where Integer Basic was loaded into in a II+ or
AppleSoft
was loaded into in a II.
I think you're mistaking the 16k card for a controller card. Apple made
a
couple of different 16k cards and a few hundred different ones were made
by
third parties but they all did pretty much the same thing.
Integer Basic was Woz's work
AppleSoft was Microsofts work. Some Applesoft ROMs had the Microsoft
copyright printed on them.
