Well, Nels, today is your lucky day.  I've just posted a .dsk image up to Asimov (should be available after whoever manages Asimov moves it out of Incoming).  

But, on the off chance you've not been checking c.s.a2 every day for the last four years since posting this request, I'll drop ya a line too.  :)

I'm continuously surprised at the stuff I'm finding while imaging my disk collection (1200+ floppy sides and counting... ways to go still). This one was unusual enough that I happened to search on it, and viola, there's your request.
