I think I found the problem.  There seems to be a "compatibility"
problem with P8CDA.SYS.

I made the disk image for the demonstration.  Then, some time later I
decided to add P8CDA and SoftSwitch to the image.  Apparently I didn't
check compatibility at that point.

If I deactivate P8CDA all works fine.  This may take a little more
research.

Thanks to everyone who tried the code in different hardware to verify.
