Understanding the Apple II by Sather has some good stuff, particularly about the video changes.
Apple II reference manual has rev 0 to rev 1 differences, some of which were functional
I have some rev 0 to rev 1 notes on my website

Note that there were some cosmetic changes made during the rev 0 production run (not all rev 0 boards are exactly the same). I would expect that this might be true of other revisions. At least some of the rev 3 boards, if not all, had an error in the artwork, requiring a wire on the back side of the PCB to be installed during production.
