HELP: Advanced Logic Systems CP/M Card and IIGS (Please Read)

I have an Advanced Logic Systems CP/M Card that I would like to use in
my Apple IIGS (ROM 01).  I also have the four diskettes from ALS with
CP/M Plus Rev. 3.01A for the CP/M Card.

What I don't have is any documentation for installing or using the
card.  If someone could give me a leg-up, I'd really appreciate it.

Which slot should it be plugged into?

What Control Panel settings should I use?

Is there a bootstrap program that I need (other than the CP/M disks
from ALS)?  If so, where can I get it?

Is there a key combination that triggers the CP/M mode?

What are the answers to all the questions I should have asked if I
knew any better?

Don't be afraid of insulting me.  Make your explanation as detailed as
you think may help.

If I can get CP/M to boot, all I need to do is re-learn everything
about it that I have forgotten over the years.  :)
