Sounds like it's likely toast since you've checked out almost ecerything.
Only other possibility I can think of is if you're getting a bad contact
between the card and slot. I've never run into that one but the recomended
way seems to be to pull the card and clean off the fingers using a pencil
eraser. Sort of a long shot but about the only one left.
