What Mark wrote is probably the problem.
When I bought two UniDisk 3.5s for my //c, one of them wasn't ejecting
properly. I opened it up and discovered that one of the eyehole
mountings was bent (The ones (4) on the sides of the housing which hold
the internal mechanism, whatever the proper name is.) It was probably
from rough shipping.
If this is the problem with yours, it's easy to bend it back to center
with a small pair of needlenose pliers.
