There is a shareware program called Chameleon which can copy files
between ProDOS, DOS 3.3, Pascal and CP/M disks.

In addition, Apple's own "Apple II System Utilities" can transfer files
between some of these file systems (I don't think it supports CP/M
beyond getting a catalog listing).

I don't know whether Chameleon can format disks - I doubt it.  I'm not
aware of other utilities which can do this (other than something that
would presumably run under CP/M).
