Try cleaning the heads (buy a 5.25" head cleaner disk kit).

Also try to get a copy of the Prodos system disk as the format program on
there should give you a more useful message (such as "write protect error"
or "disk speed too slow/fast".

If the problem is with write protection and you are sure there is a notch
cut out of the disks, then the sensor in the drive may be broken.

If the speed is the issue, it can be adjusted with a screwdriver, see the
Apple II FAQs for more information.
