Roland,
You are most likely using a newer (PowerPC era) Macintosh to make the
disk images.  Unfortunately, Apple changed the floppy drive hardware
several years ago so that writing ProDOS disks was less reliable, even
though the process *appears* successful on the Mac.

The solution appears to be to find a pre-powerPC Mac to do the same
thing with.  If this is not viable, you might consider buying system
disks from Mike Westerfield at the ByteWorks (www.byteworks.org) and
then go from there.
