Were there ever keyboards on the Apple II Plus that were permanently fixed
to require Control-Reset, instead of having a slide switch to choose?

I've never seen one. Of course, the earliest ones didn't have a
switch--they didn't require Control.

I've always wondered if that change would ever have happened, had the
reset key not been right above the return key :-)

I'm sure that explains it. Apple must have been embarassed by all
the articles explaining how to put a stronger spring or an O-ring
under the Reset key. ;-)
