I've done it, and it isn't that hard.  Using the information in "Beneath
Apple ProDOS," by Don Worth and Pieter Lechner (which I have in my hand
right now), and a program I wrote with Apples EDASM assembler (or you could
use other disk utilities that access individual sectors), I could extend the
root directory indefinitely.  The only problem, which wasn't a problem at
all, was a beep and an error message before the op system responded to my
request.  I initially tried this experiment in an attempt to be able to hide
files from prying eyes.  It worked.  A catalog command would only show what
was in the 51 file root, but if I typed in the command of the file only I
knew was in the extended directory, it ran after the error message.
Programs, like Appleworks, responded similarly.  It really wasn't all that
hard to do.  I may still be able to get my hands on the program I wrote from
the friend who has my old machine and files.  This worked for me in ProDOS
on both a IIC and IIGS, ROM 01 and 03.
