There is a nationwide organization (Alliance for Technology Access) that
provides computer labs for use by people with disabilities.
About a year ago, I was contacted by a fellow Apple II fanatic who had a
dozen or more Adaptive Firmware Cards to donate, and at that time, I
contacted the central ATA offices and they had no interest in the AFCs.
It's quite possible that some of the individual ATA sites may still be
using Apple IIs, and if they are, donations should go directly to that
local center.
The Alliance for Technology Access has labs in most states, and you can
find a listing of those at the ATA's web site:

The Red Cross is another organization to contact, as are local programs
that provide services for those with Cerebral Palsy.
Aside from those suggestions, I really have no other 'inside scoop' on
which organization could best use Adaptive Firmware Cards.
I've had conversations with people with severe disabilities who are unable
to speak, but could through the AFC and a speech synthesizer, so I know
just how miraculous these cards are. I wish I knew who was still using
them, but I just don't anymore...
