Probably the same way any new piece of software gets on disk. The creators
of ProDOS were working under some other DOS-- maybe DOS 3.3. The creators of
the first DOS probably used tape to store programs. ...

You want to type in ProDOS? (You can find some listings on Ground and the
ACN sites. I don't know whether anyone has placed a disassembly listing of
ProDOS on-line.)
Regarding the status of ProDOS: it is no longer licensed by Apple; but, it
is not public domain, either. (And, if you really want to type it in from a
listing, Apple won't complain.)
