Sounds like either a Videx Videoterm or Ultraterm 80 col. card.
Putting a video card in an //e with an apple extended memeory card/80
col card is not all that unusual. I do it myself. The videx card has a
lot neater modes than the apple card, but the apple card is required
for some brain damaged software.

It won't do well in slot 5 though. And it won't coexist in an
unenhanced //e either. Read page 142 of the //e technical reference
manual for info on how it all works.
