Need Help Burning an EPROM (Micromodem II)

I just purchased two Hayes Micromodem II cards with two Microcouplers.
One of the two cards is missing the firmware. Each of the cards
function as long as the ROM is on it. I was wondering if someone could
recommend a source who would take my original ROM and make a copy of it
onto an EPROM. This should be legal seeing I physically have the card,
but someone swiped the ROM from it. I'm willing to pay for the EPROM,
time, and shipping.

I don't know exactly what type of ROM it is, but it's a
mass-manufactured ROM (no glass window, so it's not an EEPROM). So
here's the info from ROM as it's etched on top:
