DIP switch settings for Apple II High Speed SCSI card?

I seem to have somehow misplaced my user's manual for my Apple II High
Speed SCSI Card.  Could someone who has the manual do me a big favor and
type up the guidelines for setting the DIP switches on the card?  In
particular, I want to know how to turn off the card's DMA.
