Go to Monitor, 'CALL-151', then *Cx0AG where x=slot with card. 
Or, boot ProDOS and attempt to format the drive in slot, 
Or, run a slot identifying program. Any of these should tell 
you something. And as always, reset all socketed chips.  8^)
