Motorola. Obsolete now. Get one from an old disk drive.
This one will not blow from a reversed cable.

This is also called XR2003. Made by TI, Exar and some other companies.
Very common chip, try to get one from an old printer.

After replacing one of the analog chips the drive has to be re-aligned.
The '125 and 2003 can be swapped without any adjustments.
