You might want to have a look at the ProDOS/HFS Partition Table Generator I
threw together a few years ago, available here:
http://alfter.us/computers/apple2/software.aspx
You tell it what types and sizes of partitions you want to include; it spits
out a partition table that can be cat'd together with images of partitions
to make a disk image. You can then burn this image with whatever software
you have for that purpose.
