Most likely that's a GS/OS specific program; check the 'Get Info'
from the Finder to get the filetype and report back. As you said
things weren't working from ProDOS 8, I figured (correctly, it seems)
that it was the more complex case of non-P8 apps. Yes, I covered all
the possibilities, which leads to longer, but more correct posts--
you're not helpful in the least if you're helping the wrong
problem. Too bad some people can't stand legitimacy or fully useful
information around here.
To make a GS program self-booting, use Apple's Installer to make an
800K boot disk without the Finder. Any GS/OS or ProDOS 16 application
copied to the 'System' Folder with the name 'Start', or alternatively
to the root (base) directory with a name ending in '.SYS16' will be
run at boot. See the csa2 FAQ's reference on System 6 files for what
you can safely trim if you're low on space at

Most of that advice also applies to System 5.0.4, which is slightly
smaller if you don't care about stuff like accessing Mac disks (quite
unnecessary for most games), and might do just as well for a
selfbooting 3.5" disk. For a quite stripped 5.0.4 disk (warning: do
NOT use that disk to run any desktop programs, but that's unnecessary
