I've seen flaky DRAM cause problems for DMA disk controllers.  The RF went 
through several PAL and EPROM revisions to fix issues in that area.

Can you run without the expansion memory or try a different card?  Also, try 
switching to polled transfer mode (DIP switch setting, IIRC).
