Thank you very much, everybody. I found the game by searching at
    ftp://ftp.apple.asimov.net/pub/apple_II/images/games/file_based/
for "Bert" and found the file:
    Cubit_Q-Bert_QBertOriginal_Mr.Cool.dsk.gz
I tried it on my emulator and it contained various clones of the
"Q-Bert" game. I tried them all and discovered that "Mr. Cool" is what
was called on my disk "Frozen Land".
So now I have it again. Thanks to all!
