If you're running DOS 3.3, the command is
INIT {name},Sx,Dx
{name} insert the name of the startup program
Sx - insert slot number, on 5.25" disks it's usually S6
Dx - insert drive number, either 1 or 2
If you are running in ProDOS you will need to use the system utility program or
something similar to format your disk.
