The card is labled:
BCD Associates, Inc.
Okla. City
(c) 1980
Made in USA
Model 400
SN 000474
It has 7 chips labeled:
IC1 74LS14
IC2 6520 (not 6502)
IC3 7416
IC4 7416
118-5
107-5
107-5
It has 8 shielded cables coming out of it labeled:
Cassette In (audio)
Computer Video (phono)
Channel 1 (phono)
Channel 2 (phono)
Addressing code (phono)
Player Video (recessed flush male twist-lock)
Audio in (audio)
On top of the card there is a connector such as that which connects the
card to the Apple II. It differs in being taller and having only 10
copper connectors on it.

Checking through some ancient computer mags did not turn up
anything. Evidently, the card is an A/V controller intended for handling
multi-media classroom and business presentations. Possibly, it is just
part of a larger A/V presentation system which includes video tape, sound
cassette, audio amp, and slide projector.
If no one on csa2 turns up a specific answer, you might try taking
your card to a local university's Instructional Technology department.
Someone may recognize it. At the least, you'll be able to look through
their collection of catalogs and manuals.
