The card on the photo does not contain any ROM. There are only two bus drivers and a quad NAND gate.
The D25 connector gets -12V (from pin33) but not +12V (would be pin 50).IO Select is used ($Cnxx) but Device Select is not. As each '368 buffer has 2+4 channels, you either have 8 bit out and 4 bit in, or the other way. 6 bit I/O is also possible but unlikely because all eight data lines are used.
