My vote is for Apple's own Apple II Ethernet card.  Release dates were
promised repeatedly by Apple, the last of which had release of the product
supposedly coinciding with the release of system 6.0.1.  It was even offered as
a product in Apple's own catalogs sent to educational markets.  It was never
released.
