Check the ribbon VERY CAREFULLY.
You have established that the pins are firing, so it isn't the
printhead-paper distance (you managed to print on the second page of an
NCR form).  And you've established, through the scratch-test, that the
ribbon is OK.

Sometimes, when you properly insert the ribbon into an IW-II, IW-I or
Apple DMP, the ribbon will catch on the top of the printhead.  It will
appear to be properly positioned, but will in fact be bunched up *above*
the printhead.  Generally, you can fix this problem by turning the ribbon
advance knob on the ribbon cassette; you should be able to see the ribbon
drop into position ahead of the printhead.  If the ribbon is badly worn,
it will sometimes even twist in front of the printhead, and the pins will
miss it entirely.

Just something else to check out...
