I have the same card installed in my //e with a McT SpeeDemon accelerator.  The card auto formats when the accelerator is turned OFF, but does not auto format when the accelerator is turned ON (you still can manually format it).

This definitely is a consequence of the accelerator, either because it interferes with the cards execution of its ROM on boot and/or it causes some sort of memory conflict.  Other than turning off the accelerator, I haven't found any way around this.  But having a 1 meg ram disk is really great for file transfers, archiving, de-coding, etc.
