Is there any good documentation on the web describing the details of
the Apple II disk controller? I know Woz considered it one of his
favorite designs and that it was extremely simple, but I haven't seen
any schematics or circuit descriptions.

Actually, if the design was ever described in old Byte magazines, I'd
appreciate a pointer there, too.
