Note that only one specific version of AppleWriter is freeware: version
2.1 (which runs under ProDOS and requires an Apple //e, //c or IIgs).
All previous versions (including all the DOS 3.3 disks) are still
commercial products (but unavailable) and may not be legally copied.

Almost certainly. COPYA will fail if there are any bad sectors on the
disk. If you are attempting to copy one of the really old versions of
AppleWriter (which is not legal), it might even be on a copy-protected
disk.
AppleWriter 2.1 should copy with COPYA unless the disk is damaged.
