Mystery Card `97

Can anyone identify this card:

Apple II expansion card
"MICRO
WORKS" in a Star-Wars font as part of the etching
"DS-65" below that
Three potentiometers, labelled "B", "C", and "W" (brightness, contrast, width?)
2708 EPROM near the edge connector
40-pin *empty* socket near it
Three-pin output connector, V0, GND, and V1
Two-pin jumper above the 2708, labelled "P S" or "P 6".

It's most likely a video card, I understand, but I would like to know how
to USE it :-) and also what's supposed to be in the 40-pin DIP socket
spanning "Row G" on the card's layout. What could this card do? How to I
connect it to a monitor?
