Ben,

Make sure you are holding down the Open-Apple, Option key, and
Control key while you press and release the reset key. Sometimes,
when I'm "sloppy" about this sequence I end up not activating
the self-test. If you doubt that this is what you need you might
want to try holding the Open-Apple and Option keys down while
you  boot the IIgs.

Of course, since you are getting the 911 error which indicates
a problem with the ADB controller, you may not be able to
activate the self-test since the keyboard input is controlled by
the ADB. This is just an idea though. I'm not an hardware expert.
