A possible problem: some OS feature in some file implemented since
System 5.0.4 has been damaged. Maybe the Icon file(s)? Something
the system scans while mounting volumes.

Since 5.0.2 will let you in, do this:

Boot system 5.0.2
Copy all essential files off the volume onto floppies (or tape, etc.)
Partition and format damaged hard disk under system 5.0.2
Boot hard disk
Stick in System 6.0.1 installation floppy
Run install program
Upgrade drive to System 6.0.1
Restore files from floppy EXCEPT critical system OS files

That should work.

If you don't want to format the drive, a more conservative approach
would be to delete all System 6.0.1 files from the HD under the
System 5.0.2 boot, then try to reinstall System 6.0.1 from floppy.

A little less surefire, but a lot less trouble.
