Q: Unknown Cards ???

I have this card that says:

Hayes MicroModem II   by Hayes Microcomputer Products

can't find a date on it. It's about the size of a super
serial card, has one LED on it,
and it only ends with a 9-pin connector (kinda like on the Disk II
card) on the keyboard end that looks like: 
               oo
               oo
               oo
               oo
                o

It has 2 large orange squares at the bottom.  ?? Have no idea

My question is- is this a modem, and the pins went to
a short ribbon connector that went to phone-jack inputs?

Or was this only a controller card for an external modem?
