Sounds to me like a combination of things...

1) Clean off the existing grease from the leadscrew, then apply a TINY
   amount of white lithium grease (Lubriplate works good)

2) Look for the TRK000 sensor... That's the black plastic block that
   is mounted on a small vertical PC board, and into the middle of which
   a flag on the side of the head assy passes when the heads are at TRK000.
   Clean out any dust you find here.

Another possibility is a bad TRK000 sensor :/
