Need help to Identify this Apple2 Eprom board and what the selector jumpers are!

   I found an old UNITRON EPROM WRITER for the Apple 2 the other day.   There 
is no other information on the card, and it has a 16pin programming ID socket, 
the old Multiflex programmers had a 14pin ID socket, so my old Eprom Docs are 
useless with this board.
 
   Does anyone have the docs/ID pinouts/software for this particular Eprom 
Programmer?  Its a BLUE PcBoard, not green.
