You need two cables.  Your nearest Apple dealer should be able to order
them for you.  I suppose you could try to make your own, but I don't know
the pinouts.
 
You need the following Apple Part #'s:
 
590-0620  VID I/O CABLE
590-0625  RGB VID CABLE
 
As for software, if you have a GS, VideoMix is part of the System software.
