The best one I've found it Security password v2.0. It's available at 
ground.isca.uiowa.edu in folder /apple2/apple16/gno. Look for 
password.200.shk or the binscii file. It has nothing to do with gno so I 
don't know why it got placed there.. Below is the info section from the 
binscii file...

This adds password protection to your GS/OS boot volume to prevent
unauthorized booting.  It is an alternative to the 'SetStart' Control
Panel that is part of the System 6.0 package.  Version 2.0 adds
Default Launcher path as well as startup files for both pathnames.
Boot key feature, a guest password, sounds effects, and many other
options were added at the suggestion of the beta testers.  Full
documentation included.
